CARE PLAN TEMPLATE

The user can access the Care Plan template by going to the Admin menu » CarePlan Template. Here the user can add or update the Care Plan library such as Body Systems, Problems, Care Goals, and Interventions.

Body Systems - Indicate the part or location of the identified problem
Problems - Templates that indicate the problem of a patient
Care Goals - Templates to achieve according to the identified problem
Intervention - Templates that specify the required care towards the goal

The flow of the Care Plan:

Problem » Body System » Care Goal » Intervention

Identify the problem
Narrow down the Body System Affected
Plan the Care Goal
Select the proper Intervention

The template follows the Problem » Goal » Intervention flow:

1. Problem requires at least 1 associated Body System for it to be created. The user can select a Body System from the drop-down.

2. Care Goal requires at least 1 associated Problem for it to be created. The user can select a Problem from the drop-down.

3. Intervention requires at least 1 associated Care Goal for it to be created. The user can select a Care Goal from the drop-down.

Care Plan templates other functionalities:

A default list of items for the templates is available in the system.
All items on the templates can be manipulated.
1. - Add
  - Edit
  - Delete
The user can search templates according to the selected Care Plan template library from the drop-down list.
Care Plan templates can be associated with data mapping.

Note: One problem can be associated with two Body Systems and multiple Care Goals, wherein a proper Intervention is selected.
