This guide provides step-by-step instructions for creating a recertification order when a patient’s Resumption of Care (ROC) falls within the final five days of their current 60-day certification period. Our Home Health Centre software includes a streamlined workflow for this specific scenario, ensuring compliance with CMS guidelines while saving you significant documentation time.
Before you can initiate a recertification order in this scenario, it is essential that the Resumption of Care (ROC) OASIS is completed first.
Required Status: The ROC OASIS must be, at a minimum, validated without any errors. For best practice, it should be validated, locked, or exported.
System Safeguard: If you attempt to create the recertification order while the ROC OASIS is still in progress, a helpful pop-up window will appear. This message explains the requirement to complete the ROC first and provides a direct reference link to the relevant CMS website, ensuring you always have the information needed to remain compliant.
Navigate to the Patient Summary page for the relevant patient.
Locate the Resumption of Care OASIS, which may currently be in progress.
Complete, validate, and lock the ROC OASIS to ensure there are no errors.
Once the ROC OASIS is locked, proceed to create the recertification order. You will notice that the warning pop-up no longer appears, indicating you can proceed.
Before the Recertification OASIS is created, you will see new checkbox options designed to accelerate the process:
Prefill from previous SOC/ROC/Recert Oasis
Prefill POC from previous
Select these checkboxes. This is a key feature that allows the system to build a nearly complete Recertification OASIS by pulling in the answers from the ROC you just completed.
With the information prefilled, review the Recertification OASIS.
You will need to manually complete the specific GG items that are not carried over, including:
GG0130 Self-Care
GG0170 Mobility
Finish any other required fields and finalize the OASIS as you normally would.
This streamlined process provides flexibility for how you export the completed assessments.
Navigate to the OASIS export page.
By default, you will see that both the ROC and Recert are available for export together as a single, combined set.
For more flexibility, a button labeled "Show Recertification Oasis for ROC/Recertification" is also available. Clicking this allows you to view and export the ROC and Recert OASIS assessments separately.
Note: The visibility of this button may depend on your specific user role and permissions.
