The Collaboration Note is made by a System Administrator and signed by the assigned discipline or physician. The user will find the Collaboration Note on the patient’s chart under a Certification Period.
This is created during the visit to the patient, and an assigned discipline will call the office for the following reasons:

Missed Visits
Change of Physician
Update Care Plan
Collaboration
Others
These options are located at the Reasons field.
Status - shows the current state of the document, whether it's Not Started, In Progress, etc.
Clinician - displays the name of the discipline that is in-charge of this communication note.
Physician - displays the name of the physician that is in-charge for this collaboration note.
Caller Information is the details of the caller who requested the collaboration note.
Time - captures the same time when a communication note was created.
Internal - when ticked this is for office staff or disciplines only, this will not show on the surveyor’s end.

Comments - The user can select any template for the comments by clicking on the “Template” icon. Collaboration Notes Templates are set under the Admin Menu.
Patient Alerts - can act as reminders for the clinicians.
Added Alerts will show under the Alerts section of the Patient's Chart.
When an alert is tagged as resolved, it will then lessen the Patient's Alerts count.
The user can also make a Collaboration Note through the Calendar — this way the date of the Collaboration Note will be automatically set.
Left-click the green area in the Calendar and then select the Collaboration Note option.
