If the administrator enabled an active form, it would appear after the user accepts the agreement from the EULA page. This is a required form. The user cannot proceed if they do not fill-up the form.
Viewing the submitted form as a user can be done by going to the user's Menu » User Forms. A pop-up with the list of submitted forms will be shown. This will provide the user an option to view all of the forms the user submitted, or to fill up another form.
The photo below shows how it will look if the user or administrator decides to view all the forms submitted. All the forms will be sorted out, depending on what form was filed. Both the user and the administrator have the option to print the forms submitted. All submitted forms can no longer be edited.
Viewing the submitted form as an administrator can be done by going to the Agency Menu » User. Clicking the icon under the Form column beside the user's name will open a pop-up that will show all the forms that the user submitted.
