Requirements: All field users, case managers and clinical administrators can post information and instructions and make changes to this form if they have a HHC Ultra user’s account and have access to these sections.
The Pathways Tab is integrated into each patient’s record to let clinicians supply instructions and teachings to the care staff every time they visit a patient.
Go to the patient summary page and click on the “Pathways Tab” on the upper part of the page.
The user can leave instruction/s to each plotted visit by clicking the Insert Instructions button. The number of visits depends on the plotted frequency of the patient’s visits.
Step 1: Click first on the diagnosis on the left side then the pathways and instructions will show on the right side. Clicking one instruction will automatically add it to the narrative box at the bottom.
Step 2: The user must click on “Save” when done populating the pathways instructions. The saved instructions will show below each visit date for reference.
The instruction/s or pathways will then be available in the visit note of the care staff which is accessible through the Clinical Guide.
The Instruction that was set in the Pathway Tab can be copied over to the Narratives or Instructions Text Box of the care staff visit note. The care staff can also respond to the instructions by clicking on the “Reply” button. After this, the care staff has the option to copy over the instructions and the response into the visit note.
