Requirements: All field users, case managers, and clinical administrators can post information and instructions as well as make changes to these forms if they have Hospice Centre user’s account and have access to these sections.
Initial Plan of Care
Recertification Plan of Care
The Initial Plan of Care is located under a Certification Period in the Patient's Chart.
On-going tasks for the IPOC and RePOC can also be accessed in the Patient Summary page. Upon clicking the link, it will show the POC tasks that need to be completed.
Save
It is recommended that the user saves any changes made in the document.
Lock
It is recommended that the case manager will be the one to lock the document.
No changes can be made to the document after locking it.
Review/Sign
After the document is locked, all signatory disciplines must sign the document.
Update
Given that the document is not locked, click this button if the user wants the changes that the user made in the Order or NCA-HIS to reflect in the IPOC.
Reset/Unlock
Click this button if the user needs to make changes to the IPOC. The discipline who locked the document is the one who will Reset/Unlock the IPOC. This will also remove the signatures of the disciplines.
Send to Physician
Once the office marked it as sent, the physician will receive a notification through his email (if the physician’s email is associated to his login credentials) to sign the IPOC.
Receive
Once the physician signed the document, the IPOC is marked as received.
This is also used for tracking purposes.
Hide Info
This button will hide the document stamp in the upper right corner.
Another setting for this is under Admin > Agency > Document Settings > Hide Document Fields.
Creating an order for Change Level of Care
Clicking the icon will allow the user to create an order to change the Level of Care.
Viewing of the Medication Sheet
Clicking this will open a popup containing a list of the patient's medications.
The user will be given an option to print the list.
Creating a wound treatment note
Clicking the icon will let the user create the said note.
The Initial Plan of Care has eight tabs.
Some of the information on the POC auto-populates from the Order and NCA-HIS.
Only Assigned Disciplines can access the IPOC.
Orders for Disciplines & Treatments (page 1)
Case Managers and Clinicians will indicate the frequency of the visits and necessary actions to manage the terminal illness of the patient.
Immediate Plan of Care Problems (page 2)
During the initial assessment of the admitting RN or Case Manager needs to check the problems identified about the patient’s condition.
Some of the Plan of Care Problems auto-populates from the NCA-HIS to the IPOC.
The e-signatures of the disciplines will also appear on this page after they have signed the IPOC.
Certification of Terminal Illness (page 3)
The Physician can put the brief narrative on the clinical findings that states the patient is eligible for hospice care.
The case manager and physician’s e-signature will show here once they sign the document.
Interventions
Each Care Plan Problem identified on page 2 has a corresponding intervention.
The discipline needs to check the completed intervention and input the user initials with the date it is resolved.
The discipline also needs to specify the goals and targeted outcome.
Medication Orders
This allows users to view medications administered for the patient. It also shows the status of medication and the source document where the medication is added or updated.
Diagnosis
This shows the terminal illness and comorbidities of the patient. Users can add the comorbidities of the patient. This page also shows the source document where the diagnosis was added.
Disciplines
This shows the list of assigned disciplines who will complete the document. If discipline is designated as the case manager under Assigned Disciplines in the patient chart, the box for “Is Case Manager” is automatically marked.
Only disciplines that are marked as signatory can e-sign the document.
Physician
This shows the name of the physician and his role inpatient care.
Once the physician signed the document, the date will auto-populate the Date Signed column.
Only assigned physicians in the patient chart can e-sign the document.
The Recertification Plan of Care is located under the succeeding Certification Periods in the Patient's Chart.
Unlike with the IPOC, the Recertification Plan of Care only has three pages.
However, the process to complete the Recertification POC is the same as the Initial POC.
